You’ve finally managed to clear your acne, but every time you look in the mirror you’re still reminded of where those breakouts once were. The spots may be gone, yet persistent red marks remain across the cheeks, chin or jawline, creating the appearance of ongoing acne.
This is one of the most common concerns we see in clinic and, fortunately, it is often highly treatable.
These lingering red marks are typically known as post-inflammatory erythema (PIE). Understanding what causes PIE, how it differs from acne scarring and which treatments are most effective can help you achieve a clearer, more even complexion.
Post inflammatory erythema is a vascular skin condition that develops after inflammation damages tiny blood vessels within the skin.
When an inflamed spot heals, the surrounding capillaries can remain enlarged and visible. This creates flat red, pink or purple marks that persist long after the acne itself has disappeared.
Unlike active acne, PIE is not caused by blocked pores, bacteria or excess oil production. Instead, it represents the skin’s lingering inflammatory response following a breakout.
PIE is particularly common in people who experience inflammatory acne, frequently pick or squeeze spots, have sensitive or reactive skin, or have recently completed acne treatment. While it can affect all skin types, it is often more noticeable in fair to medium skin tones.
Post-inflammatory erythema typically appears as flat areas of redness that range from light pink to deeper red or purple tones. These marks usually develop in the same locations as previous breakouts and can remain visible for weeks or even months after the acne itself has resolved.
Many patients notice that the redness becomes more prominent after exercise, exposure to heat, hot showers or alcohol consumption. This occurs because the affected blood vessels dilate more easily, making the redness temporarily more noticeable.
One of the easiest ways to identify PIE is by assessing the texture of the skin. Although the colour may be obvious, the skin surface remains smooth. Many patients believe they have acne scarring when the real issue is post-inflammatory erythema. The distinction is important because the treatments for each condition are very different.
PIE is primarily a vascular concern. The skin remains smooth, but enlarged blood vessels create visible red, pink or purple marks. Because the underlying problem is vascular, treatments that target blood vessels are often the most effective option.
Acne scars, by contrast, develop when inflammation disrupts collagen production during the healing process. This creates permanent changes in the skin’s structure, resulting in indentations, pits or uneven texture. Common examples include ice pick scars, boxcar scars and rolling scars.
