Your first lip filler: how much you need, how long it lasts and whether it's reversible
Quick answer: a first lip filler is often 0.5ml or 1ml, but the right amount depends on your lips, your face and the look you want. For many people, lip filler lasts 6 to 12 months, and hyaluronic acid filler can be dissolved if needed. If you're planning around a wedding, we usually advise having your first lip filler six to eight weeks before the day, or earlier if you can.
Before a first lip filler, people ask a lot of questions online. Am I a good candidate? Is 0.5ml enough, or should I try a lip flip? A dermatologist told me it only lasts two or three months, is that true? Is filler 100% reversible? Is four weeks before my wedding too late? You deserve clear answers before anyone picks up a needle.
At NassifMD, every first lip filler starts with a consultation with our medically led team. We look at your lips and face, talk about the result you want, and only then plan your treatment.

How our experts tailor your first lip filler
Lip filler should never feel like a production line. Your result depends on the decisions your practitioner makes for you:
- The right filler for your look. We use fillers of different thicknesses. Softer fillers give subtle volume and hydration, while firmer ones add structure and definition to the shape and border of the lip. Many patients have a combination, chosen for their lip shape and the result they want.
- The right technique for your lips. Depending on your lip shape, we use either a fine needle or a cannula, a blunt-tipped tube that moves gently under the skin. A cannula lowers the chance of bruising, although bruising can still happen.
- The right amount. We build gradually for balanced, natural-looking lips that suit your face, in line with the approach Dr Paul Nassif is known for. Read more about how we keep lip filler natural.
Am I a good candidate for lip filler?
Lip filler may suit you if your lips are:
- Smaller or thinner than you'd like, through genes or ageing
- Lacking a defined cupid's bow or lip border
- Showing fine lip lines
- Unbalanced between the top and bottom lip, or uneven side to side
It may not be right for you at the moment if you have inflamed skin or a skin condition around the mouth, or if you're pregnant or breastfeeding. Tell us if you're prone to cold sores. Your suitability is decided at a thorough consultation, and if another treatment would suit you better, we'll tell you.

Is 0.5ml enough for a first lip filler?
For many first-timers, yes. 1ml is a small amount, about a fifth of a teaspoon, and 0.5ml is half that.
- 0.5ml usually gives subtle hydration and a little more shape. Many people describe it as their own lips, only better.
- 1ml can give more noticeable fullness, or room to add both volume and definition.
The amount is only part of the story. Where the filler is placed, which product is used and how it suits your face matter far more to whether lips look natural. Very thin lips may need structure along the border before volume. We'd always rather start gently and add more at your review than overfill on day one.

Lip flip vs filler: what's the difference?
A lip flip uses a small amount of anti-wrinkle injection near the top lip to turn it slightly outwards. It doesn't add volume, but it can make the top lip look fuller, especially if it seems to disappear when you smile. It usually wears off sooner than filler.
Lip filler adds actual volume, shape and definition. If you want fuller lips, a sharper border or smoother lip lines, filler is usually the better fit. Some people have both. Not everyone is a candidate for a lip flip, so your practitioner will talk you through which suits your lips.
How long does lip filler last?
Results typically last 6 to 12 months, depending on the product used, the area treated and individual factors such as metabolism. Softer fillers used for subtle hydration can fade sooner than firmer ones.
If you've heard lip filler only lasts two or three months, it's often because lips look much bigger in the first week while they're swollen. Once swelling has gone, what's left at around four weeks is your true result. Our lip filler swelling day-by-day guide shows what to expect.
Is lip filler reversible?
Hyaluronic acid filler can be dissolved with an enzyme if needed, and we only use non-permanent filler. Dissolving is a treatment in its own right, though. Lips can be swollen afterwards and more than one appointment is sometimes needed, so it's best kept as a safety net rather than a plan.
If you're unhappy with your result, we usually recommend waiting until any swelling has fully settled and discussing it with us at a follow-up appointment. If you've had filler elsewhere, our Correctional Clinic can assess it and dissolve it where that's the right option.
How far before a wedding should I have lip filler?
For a first lip filler, we usually advise six to eight weeks before the day, and earlier still if you can. Swelling has usually settled by two weeks and your final result shows at around four weeks, so this leaves time for your review and any small adjustment. Four weeks is possible, but it leaves little room if you'd like a tweak. If you've had lip filler before and know how your lips respond, your practitioner may be happy with a shorter gap.
Remember to avoid dental treatment for two weeks after filler, and plan any skin treatments early. Our bridal preparation treatments can help you plan it all.
What to expect on the day
Treatment takes around 30 minutes and downtime is minimal. The fillers we use usually contain lidocaine, a mild anaesthetic, and numbing cream can be offered beforehand. Your lips will feel plump and tender at first, and swelling usually peaks on day 2 or 3. One of our team will contact you within 24 hours, and we offer complimentary follow-up appointments for our lip filler patients.

Frequently asked questions
Is 0.5ml enough for a first lip filler?
For many people having lip filler for the first time, 0.5ml gives a subtle, natural-looking change. Some lips need more for the look you want. Your practitioner will recommend an amount at your consultation, and more can be added later if you'd like.

How long does lip filler last the first time?
For many people, lip filler lasts 6 to 12 months, depending on the product used, the area treated and individual factors such as metabolism. Lips can look smaller once the first swelling settles, which is why some people think it has faded quickly.
Can lip filler be dissolved?
Yes. Hyaluronic acid filler can be dissolved with an enzyme if needed. We usually recommend waiting until swelling has fully settled and discussing it at a follow-up appointment first, unless there's a medical reason to act sooner.

Does lip filler hurt?
Lips are sensitive, but the fillers we use usually contain lidocaine, a mild anaesthetic, and numbing cream can be offered beforehand. Most people find it very manageable.
