Lip filler swelling day by day guide from NassifMD Medical Spa Manchester

Lip Filler Swelling Day by Day: What's Normal and When to Call Us

Quick answer: lip filler swelling usually peaks on day 2 or 3, is mostly gone by day 7 and has settled by two weeks, with your final result at around four weeks. Lumps and unevenness in the first week are common. A pale, white or blue-grey patch, or pain that keeps getting worse, needs help straight away.

You've just had lip filler, you look in the mirror the next morning and your lips are bigger, lumpier and more lopsided than you expected. You're not alone. Every week, hundreds of people post photos online asking strangers "is this normal?", often because they can't get hold of the person who treated them.

This guide walks you through lip filler swelling day by day, so you know what's expected, what isn't, and exactly when to pick up the phone. If you've had lip filler with us at NassifMD, call us on 0161 877 8444, or contact our out-of-hours concierge team using the details in your aftercare information. That's what we're here for.

Lip filler swelling day by day timeline: what's normal from treatment day to week 4, and warning signs to call NassifMD Manchester

Why great lip filler starts long before the first injection

Lip filler isn't a one-size-fits-all treatment, and it should never feel like a production line. How your lips look and heal depends on the decisions your practitioner makes before and during treatment. At NassifMD, our medically led team tailors every treatment to you:

  • The right filler for your look. We use fillers of different thicknesses. Softer fillers give subtle volume and hydration, while firmer ones add structure and definition to the shape and border of the lip. Many patients have a combination, chosen for their lip shape and the result they want.
  • The right technique for your lips. Depending on your lip shape, we use either a fine needle or a cannula, a blunt-tipped tube that moves gently under the skin. A cannula lowers the chance of bruising, although bruising can still happen.
  • The right amount. We build gradually for balanced, natural-looking lips that suit your face, in line with the approach Dr Paul Nassif is known for. Read more about how we keep lip filler natural.

Even with expert technique, your lips go through a normal healing process. Here's what to expect.

Lip filler swelling stages: the day-by-day timeline

Day 0: treatment day

Straight after treatment your lips will feel plump, tender and a little numb from the anaesthetic in the filler. You may see small pinprick marks. Some people bruise and many don't. They can look uneven at this stage because some areas swell more than others. That's normal.

Days 1 to 3: peak swelling

This is the stage most people worry about. Swelling usually peaks on day 2 or 3 and is often worse first thing in the morning. Your lips can look overfilled, the top lip may stick out more than the bottom, and you may feel small lumps. If you have bruised, it's usually most visible now and can look purple or blue before fading to yellow.

Try not to judge your result yet. What you're seeing is mostly swelling, not filler.

Days 4 to 7: settling

Swelling, and any bruising, fades noticeably and your lips start to look like your own again. You may still feel small firm areas when you press your lips together. These are usually filler settling into place and soften over the next week.

Week 2: the review point

By two weeks most swelling has gone and lumps and asymmetry should have settled. This is the right time to judge the shape. If anything still bothers you, book a follow-up so we can look properly. We offer complimentary follow-up appointments for our lip filler patients.

Week 4: your final result

By around four weeks the filler has fully integrated and your lips should feel soft and look natural. This is your true result.

Lip filler lumps: what's normal and what isn't

Lumps after lip filler are one of the most common worries. In the first week they're usually caused by swelling, small bruises or filler that hasn't settled yet. Don't massage or squeeze them unless your practitioner has told you to, as this can move the filler.

  • Lip filler lumps after 3 to 5 days: very common and usually settle on their own.
  • Lip filler lumps after 1 week: often still settling. Keep an eye on them.
  • Lip filler lumps after 2 weeks or more: book a review. Lumps that last this long can usually be smoothed or, if needed, dissolved.
  • Lumps that appear months later: get these checked rather than waiting.

Is asymmetry normal after lip filler?

Yes, in the first week. Swelling rarely happens evenly, so one side or one lip often looks bigger for a few days. Most natural lips aren't perfectly symmetrical to begin with either. Wait until the two-week mark before deciding whether the shape needs adjusting.

What does lip filler blanching look like?

Blanching means the skin turns pale or white. A brief paleness during the injection itself can happen and fades within seconds. Blanching that stays, or skin that turns blotchy, dusky blue-grey or purple, is different. Together with pain that is getting worse rather than better, it can be a sign that filler is affecting blood flow. This is called a vascular occlusion.

It's rare, but it needs treating quickly, usually with an enzyme that dissolves the filler. Don't wait to see if it settles.

When to call us straight away

Contact your practitioner immediately if you notice any of these:

  • A pale, white, blotchy or blue-grey patch on or around your lips
  • Pain that is severe or getting worse after the first day
  • Heat, spreading redness, pus or blisters, or you feel feverish and unwell
  • Swelling that keeps increasing after day 3, or swelling with an itchy rash

Had filler somewhere else and can't get an answer? Our Correctional Clinic assesses and corrects filler done elsewhere, including dissolving filler where that's the right option.

Lip filler aftercare: dos and don'ts

  • Do use a cold compress, wrapped in a clean cloth, for short periods in the first 24 hours to ease swelling.
  • Do sleep on your back with your head raised on the first night.
  • Do take paracetamol if you're sore. Avoid aspirin and ibuprofen unless your doctor advises them, as they can increase bruising.
  • Don't drink alcohol for 24 hours.
  • Don't do strenuous exercise for 24 to 48 hours.
  • Don't use saunas, steam rooms or sunbeds for 48 hours.
  • Don't wear lipstick or lip products for the first 24 hours.
  • Don't book dental treatment for two weeks after your filler.

Frequently asked questions

How long does lip filler swelling last?

Swelling peaks around day 2 or 3, is mostly gone by day 7 and has fully settled by two weeks. Your final result shows at around four weeks.

How long does lip filler bruising last?

Not everyone bruises. Where it happens, it usually fades within 5 to 10 days. Choosing a cannula where it suits your lip shape lowers the chance of bruising, and a cold compress on the first day plus avoiding alcohol and blood-thinning painkillers also help.

Can I kiss after lip filler?

It's best to avoid kissing for 24 to 48 hours. Pressure on fresh filler can move it and your lips will be tender.

How should I sleep after lip filler?

Sleep on your back with your head slightly raised for the first night or two. Avoid sleeping face down so there's no pressure on your lips.

Can lip filler be dissolved if I don't like it?

Hyaluronic acid filler can be dissolved. We usually suggest waiting two weeks for swelling to settle before deciding, unless there's a medical reason to act sooner.

Who do I contact if my injector isn't answering?

If you're worried about blanching, skin colour changes or severe pain, don't wait. Go straight to A&E. For lumps, asymmetry or results you're unhappy with, our Correctional Clinic can help.

If thin lips or fine lip lines are your main concern, see our guide to treatments for thin lips and lip lines.
